School Enrollment Immunization Requirements
Children enrolling in school must either provide immunization (vaccination) records to prove they are up to date with their immunizations or have an exemption on file with Southwestern Public Health.
If a child does not have their immunization records, they will be considered unimmunized. To meet school enrolment requirements, the child would have to receive all required vaccines. There is no risk in being re-immunized in the case of a child who has already been immunized but has no records. Immunizations can be done through Southwestern Public Health.
Alternatively, the parent or guardian of the child would have to complete a non-medical exemption form to provide a Statement of Conscience or Religious Belief. This would exempt the child from receiving immunizations. In the future, the child could still receive any vaccines that are due or overdue based on the Ontario Immunization Schedule.
